Delaware Youth Soccer Association will issue an insurance certificate each August for the soccer seasonal year to affiliated clubs in good standing.
When clubs or teams rent or reserve fields or facilities, the owner often asks for proof of insurance from the organization. The form used to provide such proof of insurance is a Certificate of Insurance. The Certificate of Insurance can be obtained from Delaware Youth Soccer Association (DYSA) and will take 2-3 days to process, so please plan ahead!
Only the President or Executive Director of an affiliated club may request a Certificate of Insurance from our office.
Please note: This certificate is for games and training sessions involving the affiliated club ONLY. Insurance coverage does not extend to any events not sanctioned or acknowledged as approved coverage by DYSA. Certificates will not be provided unless your club is affiliated with DYSA and in good standing.
Your submission of this request acknowledges that your organization is in full compliance with all DYSA COVID-19 Return to Play policies. In addition, your club is in full compliance with any COVID-19 regulations as provided by the CDC, State of Delaware Department of Health and any local government.
Lost or additionally insured certificates may be obtained from Delaware Youth Soccer Association for a fee of $25.00 per certificate
