INSURANCE CERTIFICATES
Delaware Youth Soccer Association will issue an insurance certificate each August for the soccer seasonal year to affiliated clubs in good standing.
ADDITIONALLY INSURED
When clubs or teams rent or reserve fields or facilities, the owner often asks for proof of insurance from the organization. The form used to provide such proof of insurance is a Certificate of Insurance. The Certificate of Insurance can be obtained from Delaware Youth Soccer Association (DYSA) and will take 2-3 days to process, so please plan ahead!
Only the President or Executive Director of an affiliated club may request a Certificate of Insurance from our office.
Please note: This certificate is for games and training sessions involving the affiliated club ONLY. Insurance coverage does not extend to any events not sanctioned or acknowledged as approved coverage by DYSA. Certificates will not be provided unless your club is affiliated with DYSA and in good standing.
Lost or additionally insured certificates may be obtained from Delaware Youth Soccer Association for a fee of $25.00 per certificate
If a club would like to obtain additional certificates (field use, gym use, etc.) please have the club president send request to the Executive Director at, Executivedirector@dysa.org. Please send:
- Name of the Facility to be insured
- Address of the Facility to be insured
- Mail a check for $25.00 to DYSA, 222 Benjamin Blvd., Bear, DE 19701 (per certificate)
Requests will not be taken from individual teams, coaches or team managers.
