DYSA Insurance Claim Procedures

(1)  Club representative needs to complete a DYSA Accident report and fax to DYSA secretary at 328-9235.

(2) Have parents complete a claim form from the Bollinger Insurance link.

(3) Once the above steps are complete, mail the claim form, receipts and other documentation to DYSA, PO Box 11045, Wilmington, DE 19850, Attention:  Laura Sturdivant

(4) DYSA will complete their portion and forward to Bollinger Insurance for processing.
